The complete guide to reversing type 2 diabetes in India

By Col. Gautam Guha · Updated 2 Oct 2026

Short answer

Reversing type 2 diabetes in India means reaching an HbA1c below 6.5% without glucose-lowering medication, sustained for at least three months. It is achieved through fat loss, reduced refined carbohydrate, deliberate protein, daily movement, resistance training, seven hours of sleep and stress work — coordinated with your doctor.

Step 1 — Know your starting point

Get HbA1c, fasting and post-meal glucose, a lipid profile, fasting insulin or C-peptide, liver enzymes with an ultrasound, vitamin D, vitamin B12, TSH and a creatinine with eGFR. Measure your waist. Note how many years since diagnosis and every medication with its dose.

Reversal odds are best within six years of diagnosis, with a preserved C-peptide, visible weight to lose, and no insulin or only a low dose.

Step 2 — Fix the plate

Halve the refined carbohydrate volume, put 25–30 g of protein in every main meal, fill half the plate with vegetables, and eat protein and vegetables before the grain. Swap white rice and maida for millets, jowar, bajra or a smaller portion of whole grain. Remove sugar, jaggery, honey, juice and packaged snacks during the reversal phase.

Step 3 — Move, sleep, and lower stress

Ten to fifteen minutes of walking after each main meal, 7,000–8,000 steps a day, two resistance sessions a week, seven to eight hours of sleep, and a daily ten-minute slow-breathing or yoga practice. These four are not optional extras — they are why identical diets produce different results in different people.

Step 4 — Track and taper safely

Log fasting and post-meal glucose, weight and waist weekly. Retest HbA1c at twelve weeks, never sooner. Take the data to your doctor and let them make every medication decision. Never taper on your own — doses correct at an HbA1c of 9% can cause hypoglycaemia at 7%.

Expect fasting glucose to respond in weeks two to four, HbA1c to move at the twelve-week retest, and medication conversations between month three and month nine.

Step 5 — Make it permanent

Remission is maintenance, not a finish line. The DiRECT follow-up is unambiguous: those who kept the weight off kept remission, those who regained it relapsed. Build the version of this you can run for twenty years, not the strictest version you can survive for twelve weeks.

Related questions

How long does diabetes reversal take?
Most people who achieve remission do so between three and twelve months. Fasting glucose improves within weeks; HbA1c needs at least twelve weeks to reflect change.
Do I need a programme to reverse diabetes?
Not strictly. Everything needed is in this guide. What a programme adds is accountability, data interpretation and someone noticing when you drift — which is why coached programmes outperform self-directed attempts in trials.
What if I do not achieve full remission?
Partial improvement is still a major win — fewer medications, lower complication risk, better energy and weight. Remission is the target, not the only acceptable outcome.
