Signs of insulin resistance — the problem behind the numbers

Insulin resistance shows up as an expanding waist, darkened velvety skin on the neck or armpits, skin tags, heavy sleepiness after meals, high triglycerides with low HDL, fatty liver on ultrasound, PCOS, and blood pressure creeping up — often years before blood glucose becomes abnormal.
The visible clues
Insulin resistance is the engine underneath type 2 diabetes, PCOS, fatty liver and much of metabolic heart disease. It becomes visible long before it becomes measurable on a sugar test.
- Waist above 90 cm (men) or 80 cm (women), even at normal weight
- Acanthosis nigricans — velvety dark patches on neck, armpits, groin or knuckles
- Skin tags around the neck and underarms
- Overwhelming sleepiness 45–90 minutes after a carbohydrate-heavy meal
- Intense sugar cravings in the late afternoon and after dinner
- Difficulty losing weight despite genuine calorie restriction
The blood clues
Ask for a lipid profile, fasting insulin and liver enzymes alongside your glucose tests. A triglyceride level above 150 mg/dL with HDL below 40 (men) or 50 (women) is a strong marker. A triglyceride-to-HDL ratio above 3 is one of the cheapest and most useful signals available in India.
Fasting insulin above roughly 10 µIU/mL, or a HOMA-IR above 2.5, indicates the pancreas is working overtime to keep glucose normal. Raised ALT with a fatty liver on ultrasound points the same way.
Why it precedes diabetes by a decade
As cells stop responding to insulin, the pancreas simply makes more. For years this brute-force compensation keeps blood sugar in the normal range, and every test you take comes back reassuring. Diabetes appears only when the pancreas can no longer keep up.
That is the tragedy and the opportunity: the decade of silent, compensated insulin resistance is the decade in which the condition is easiest to reverse.
What reverses it
Losing visceral and liver fat, building muscle through resistance training, reducing refined carbohydrate load, sleeping seven hours and managing chronic stress. There is no supplement equivalent. Muscle is the largest disposal site for glucose in the body, and it is the one you can actively grow at any age.
