Can you stop diabetes medication safely?

Diabetes medication can often be reduced or stopped once lifestyle change brings glucose into range — but only under your treating doctor's supervision, guided by home glucose data. Sulfonylureas and insulin are usually tapered first because they cause hypoglycaemia; metformin is typically the last to go.
The order of tapering
Doctors generally reduce hypoglycaemia-causing drugs first — insulin and sulfonylureas such as glimepiride and gliclazide — because as your diet changes, these are the drugs that will drive you low. SGLT2 inhibitors and DPP-4 inhibitors come next. Metformin is usually last, and some people choose to remain on it for its other benefits.
What justifies a taper
Two weeks of fasting readings consistently under 110 mg/dL, post-meal under 140, no hypoglycaemic episodes, and a documented change in diet and activity that you are confident you can sustain. Bring the log to your doctor — a data-backed request gets a very different response from a hopeful one.
Never do this
Do not stop insulin abruptly, especially if there is any possibility of type 1 or LADA — this risks diabetic ketoacidosis, which is a medical emergency. Do not stop blood pressure, statin or kidney-protective medication because your sugar improved; those are separate decisions.
