Does intermittent fasting reverse diabetes?

Short answer

Intermittent fasting helps reverse type 2 diabetes by lowering insulin exposure and clearing liver fat, but only when the food inside the eating window is low in refined carbohydrate. Early time-restricted eating — a 8-10 hour window finishing by 7:30 pm — works best. People on insulin or sulfonylureas need medical supervision.

Why it works

Every eating episode raises insulin. Fewer, tighter eating windows mean longer stretches of low insulin, which is when the body accesses stored fat — including liver fat, the direct driver of high fasting glucose. Fasting also improves the circadian alignment of glucose handling.

Which protocol

Start with 12 hours and shorten by one hour a week to 10 or, if comfortable, 8. Keep the window early: 9 am-7 pm beats 12 pm-10 pm for the same duration, because insulin sensitivity is higher in the morning. Skip breakfast or skip dinner — but do not skip both and then eat a mountain at 10 pm.

Safety first

If you take insulin, sulfonylureas (glimepiride, gliclazide) or glinides, fasting can cause hypoglycaemia — dose adjustment must come from your doctor before you extend the window. Fasting is not appropriate in pregnancy, active eating disorders, type 1 diabetes without specialist supervision, or in the frail elderly.

The common failure

A perfect 16:8 window filled with poha, biscuits, fruit juice and two sweet chais will not reverse anything. Fasting amplifies a good diet; it does not replace one.
